      • Fall of worker seven storeys from scaffold during demolition

      • Consultant to analyse effectiveness of risk assessment and management practices in Singapore , for the Workplace Safety and Health Council
  • Developed course modules on accident causation, investigation, and prevention for project management courses by IES, SCAL, etc.
  • Formalised and presented my ‘5A-Way’ applied to safety
  • Scores of invited talks for various organisations on workplace safety and risk management, among them, three keynote talks to MRT:
